Documenting Using PowerNote

1. Choose Date and Time from Calendar 2. Complete Clinical Reason/Diagnosis for Home Health i.e. Diabetes, Wound Care Must include clinical diagnosis 2. Complete Clinical Reason/Diagnosis for Home Health i.e. Diabetes, Wound Care Must include clinical diagnosis 3. Choose Type of Service Please include Skilled nursing for medication management 3. Select Two or More of the Homebound Reasons along with diagnosis to support. ( post op weakness, Exacerbation CHF, Debility. There must be 2 homebound qualifiers 5. PowerNote is Complete! When Certification field is clicked, documentation is added to Physician’s Progress Note in Patient’s Care Connection Record once you click sign and submit. F2F power note must be cosigned by an attending.
