COMMUNITY HEALTH PARTNERSHIPS Primary care in Scotland is organised into Community Health Partnerships. Health professionals work together to improve the lives of people in their area. There are ten CHP’s across the Greater Glasgow area. Each CHP provides primary care services such as: health visiting district nursing speech and language therapy mental health nutrition services

WORKING IN PARTNERSHIP Work in health clinics, schools and homes. CHP’s will work closely with local councils, hospitals and community groups to break down the barriers and improve health of the local people.
