Criteria Categories

Culture  environmental and operational core of your organization  rooted in successful aging: growth & potential

Leadership  visible commitment to successful aging  maximizing growth and potential throughout the entire organization

Outreach  an expert in the aging field  partnerships to benefit the greater community  furthering the successful aging movement throughout the country

Team Member Engagement  successful aging principles are incorporated into team members’ interactions  with one another  with residents/members  and in their personal lives

Resident/Member Engagement  residents/members take ownership and leadership in the successful aging culture  active contribution and involvement throughout the organization

Purposeful Programming  programming incorporates the four components of successful aging  designed around the strengths and areas for growth within the population  data driven  continual focus on growth

Centers for Successful Aging Qualification Checklist 18 months post-launch/debut as of September 1 st 2014 Launched in Independent and Assisted Living (if there are multiple levels of living at your organization) Successful Aging/Masterpiece Living incorporated into organization’s strategic plan Active Champion Team in place Multiple Champion Team members and Executive Director have reviewed the criteria and have developed a team approach for completing the application An estimated 80+% of criteria requirements are met At least 45% review completion (calculated by Partnership Specialist) Executive Director and at least one Champion Team member completed Foundational Training in the last 4 years (September 1 st, 2010 through September 1 st 2014) Who should apply?

 Applications accepted annually  Use criteria as a vision for the future  Identify strengths and opportunities for growth  Set goals and strategies based on CSA criteria Not ready yet?

Qualification Review  De-identified information  Review Board  MPL Advisory Board Members  Select MPL Team Members  Score of 90%

Once qualified  Certification valid for 2 years  Re-apply via a modified application process  Maintain minimum CSA standards and supporting data  Act as mentor for the network  Priority invitations to host tours and trainings
