2 Purposes of the Briefing §To understand the background of the service extension to PWDs §To introduce the implementation details and procedures of the service extension §To share the experience in delivering IHCS to PWDs §To introduce the community support services on rehabilitation

3 Background of IHCS  60 district and agency – based Integrated Home Care Services Teams (IHCSTs) were formed in the territory during the Re-engineering Exercise on Community Support Services for Elders in April 2003 by:  Transforming the then 25 Home Care and meal Teams (set up in 1999 and 2000 respectively); and  Upgrading the then 138 Home-Help Teams (HHTs) with dditional resources Only 1 HHT left in Tung Chung Area

4 Background of IHCS (Cont ’ d) §Under the IHCST, there are two service groups:  Frail cases (1 120 quota in all districts) – service provision similar to Enhanced Home and Community Care Services (EHCCS)  Ordinary cases (no fixed quota) – service provision similar to HHT. §During the upgrading exercise, IHCSTs have also taken up 37 PWD cases under frail case quota.  After Central Waiting List for elders was in place on 28 November 2003, the subject on PWDs ’ access to IHCS (frail cases) was deliberated among parties concerned.

5 Extension of IHCS (Frail Cases) to PWDs Target PWDs §Suffer from severe level of physical disabilities with care need levels equivalent to the eligibility of HSPH or C&A/SD, and §Not receiving any regular day rehabilitation services

6 Admission requirement §Confirmed to be severe impairment level by Minimum Data Set-Home Care under Standardized Care Need Assessment Mechanism (Elderly Services); and §Require a comprehensive service package by IHCS (Frail Cases)

7 Service provision to PWDs IHCSTs to provide: §Similar service provision as elder users §Care and maintenance services instead of intensive rehabilitation training and treatment programmes

8 Meetings among HKCSS, IHCSTs and related Branches on service extension to PWDs §2 March 2004 §18 May 2004

9 Support to IHCSTs Training  4 classes (each with 60 capacities) including 2 for professional staff and 2 for care workers providing a total of 240 training places conducted from end June to early August 2004  Further training for 4 classes will be scheduled in Q3 and Q4 2004/05

10 Support to IHCSTs (Cont ’ d) §Strategic alliance  Strategic alliance and sharing on care experience between IHCSTs and rehabilitation service units is encouraged  Information distribution of pamphlets and information briefs etc. were shared between IHCSTs and rehabilitation service units in June 2004 to facilitate mutual understanding of service provision  Sharing session between IHCSTs and community support services units on rehabilitation was held on 20 July 2004  SCNAMO(ES)s and PCTs will assist in strategic alliances as far as possible in the districts

11 Time schedule for implementation §Implementation date of IHCS extension to PWDs is scheduled on 16 August 2004 (Monday) §The referring procedures will be distributed and uploaded in SWD Homepage in early August 2004
