Recordable injury 3/23/07 What Happened: –Employee was using a pry bar between a 2”x4” wood block support & an I-beam. The wood block slipped out & the beam dropped causing the employee’s right hand to be caught between the pry bar & the concrete ledge. The employee received fractures to 2 finger tips & required sutures.

Recordable injury 3/23/07 Employees chose to try to position the beam to mark drill hole placement on the wall by using a pry bar instead of waiting on the crane to lift the beam.

Recordable injury 3/23/07 Corrective Actions: –Employees instructed to use cranes or other lifting devices to lift & support beams & pipe instead of pry bars. –Pinch points were covered in JHA, but employee did not consider all the possibilities of his actions.
