You or your loved one have been in the hospital and it’s time to go home. A hospital stay can be a stressful time. While you’re probably happy to be heading home, you may have plenty of questions you would like answered before leaving the hospital. Hospital to home ? ? ? ? ? ?

Planning to leave the hospital is called “discharge planning”. Discharge planning helps to make sure that you leave the hospital safely and smoothly and get the right care after that. We want to partner with our patients and their families to ensure that you have the information you need. Hospital to home

Did someone speak with you about leaving the hospital when you were a patient here? Before you leave the hospital, you may receive a number of instructions from different members of your health care team. Members of your health care team may include: Nurses Respiratory therapists Social workers Physical or occupational therapists Dietitians Doctors Did you have all of your questions answered?
