Take 5 for Safety E. Lessard January 13, 2015

Readiness for High Consequence Risks: Accelerator Operations  Implementing procedures tie responsible persons to credited controls in the ASEs (safety envelopes)  The limits in the ASE are related to tangible controls that must be present during operations  Safety committee reviews were used to address technical ESH issues, review changes and modifications of accelerators and experiments against the requirements in the ASEs and to review assumptions in the safety analysis  Operating organization structure and authorizations are addressed  Engineered controls for non-standard industrial hazards are operational  Drawings, calculations and controlled documents are signed and retrievable  Procedures and authorizations are up to date  Training is defined and up to date for each worker and user 2

Readiness for High Frequency Risks: Slip/Fall Injuries  15 Recordable Slip/Fall Injuries at C-AD, 2003 to 2014  Slip/Fall Training – contact John Maraviglia x7343 3

Readiness for High Frequency Risks: Backing-up Accidents  7 backing-up accidents at C-AD in last 3 years out of 9 total government vehicle accidents  Walk around vehicle before backing up  Double check and reposition mirrors before starting 4

Look for Hazards in Every Direction 5
