You will be only using the first two letters, S and O. The subjective is the signs and symptoms that the patient will give you, such as: “I am having pain when urinating” and “I get up many times during the night to urinate” and “my urine is brown.” The objective is what you observe, such as: Temperature: 98.6, Pulse: 62, Respiration: 16, Weight: 155, Height 5’ 5”. Using either or both chapters (6 and/or 7) create a SOAP note using five medical words discussed in this week’s reading.
