Our collaborative approach focuses on 3 key elements referred to as Triple AIM: Better Health, Better Care and Better Cost Better Health meaning . . . .managing populations to identify patients that need care and that we are engaging more patients in their health through our innovative programs. Better Care meaning . . . . Earlier intervention of patients that need care or that are not following care plans and with more coordination to eliminate redundancies . Embedded Care Teams (Aetna and the provider) working together to coordinate care and follow-up on care in between visits Together, they are not waiting for members to make appointments. They are being proactive by reaching out to and engaging members with technology and support. Better Cost meaning . . . Reducing unnecessary utilization translating into a more competitive offering: 8-15% first year savings with sustained savings So this all sounds good, But let’s hear from some of our ACO collaborators to give you a better sense of how we are truly changing the model.
