PATIENT- AND FAMILY-CENTERED CARE: Partnerships for Safety & Quality Staff Physician & Resident Physician Toolkit

Recognizing the Driving Forces

 System-Centered (SC) The priorities of the system and those who work within it drive the delivery of care.  Patient-Focused (PF) The patient is the focus or unit of care. Interventions are done to and for him/her, instead of with the patient. The patient is not viewed within the context of family or community.  Family-Focused (FF) While the family is the focus or unit of care, interventions are done to and for them, instead of with them  Patient- and Family-Centered (PFC) Patient- and family-centered: the priorities and choices of the patient and family drive the delivery of health care

1. Visiting policies specify that spouses and children are allowed to visit the intensive care units at any time. FF - SC

2. Patients receiving care at a family medicine clinic are encouraged to read their charts, even the social work notes. PFC

3. In developing a new ambulatory clinic, VA leadership invite Veterans and families to comment on the final plans for the facility's upcoming renovation. SC

4. A patient advisor who has been newly added as a member of the Quality Committee is sent a letter telling her where and when the next meeting will be held. PFC - SC

5. Patients who have received care at the VA, and their families, participate as faculty in the orientation program for new staff and providers. PFC

6. A social worker's job description states that “she/he is to identify patient and family needs for each discharged ventilator-dependent quadriplegic patient.” PF - FF

7. After reviewing the VA’s patient satisfaction survey data, the clinic changes its hours of operation to include some evening hours. PF - PFC

8. Patients who have had a stroke and their families develop a peer support program. PFC

9. A physician asks a husband whose wife experienced a complicated total joint replacement to join the residents’ noon conference to discuss what communication from physicians was helpful to him and his family and what was not. PFC

10. The policy of the housekeeping department is to ask Veterans and families their preferred time of day for their hospital room to be cleaned. PFC

11. After getting permission from their physician, patients and family members have access to the hospital library. SC

12. A multidisciplinary committee develops new educational materials about asthma for patients and their families. PF - FF
