Care Homes Health and social care services now meet monthly with care home staff New role which links care homes across health and social care We are working with residents, their families, care home staff, GPs and other health and social care staff, for “in case of emergency” planning Feedback to date has highlighted: o Better links between care homes and health services; o Improved later-life planning; o Access to individual, personalised plans; o Open conversations; o Quality and consistency of planned care. 5
