Angus Gartshore Community Mental Health Teams Transformation Programme: Supporting Recovery and Improving Flow

To support recovery and improve flow we will: Improve access to our service and standardise our assessment processes. Implement new approaches to care co-ordination. Ensure that effective therapeutic interventions are available as described in our care pathways. Improve the systems and processes that support transfer of care for those who no longer need our service. The key underlying principle of the programme is to promote a consistent culture and approach across all our community mental health teams.
