Design of a Chlor-Alkali plant –Designer was trying to avoid exotic metals normally needed –Chemicals were not reactive to plastics used in piping Had not been done before –Designer recommended a less-expensive solution –After months of deliberation, client declined Went with a much more expensive and time-consuming design Why? –This is a people problem The problem
