Appendix 3: The Standard Report From the Church Vitality Survey This Standard Report is produced for each church or church group that completes the Church Vitality Survey. It consists of a Summary Page and five Sections. For an overview of this Report, please refer to “Unit 2 - Understanding The Standard Report From the Church Vitality Survey”. ( You can also look at Appendix 2 for some examples and explanations of the various Sections of the Standard Report. Page 2: Section 1 – Summary of Responses. This contains a Table and a Graph of every question on the Church Vitality Survey except Question 1 - the name of the church. Question 2 contains the Unique ID of each person who took the Survey. If the church has assigned the Unique ID they can use this question to monitor who has taken the Survey. Page 47: Section 2 – Cross Tabulations Between Two Questions. The Standard Report contains a "Cross Tabulation Between Questions 11 and 12". It shows the relationship between the answers for questions 11 and 12. For an explanation of Cross Tabulations see Appendix 2. Page 48: Section 3 - Seven Profiles and Graphs For Entire Church. These summarize the way respondents answered certain qroups of questions on the Survey. Profiles are created by assigning a numerical value to the answers for some of the questions on the Survey. For additional examples and explanation see Appendix 2 and Unit 2 on this Training Menu. Page 56: Section 4 - Summary of Profiles For Each Respondent. This provides a Table summary of the Profiles for each person who responded to the Survey. See Unit 2 for an explanation of Section 4. Page 69: Section 5 – Summary of Profiles in Table Form. This is different from Section 4 in that it shows a table with summarized Profiles for the entire church. The first line of each Profile shows the numerical Score and the 2nd line shows the Percentage of the church that scored in each statistical category. Enhancements: Every Section of the Standard Report contains information that has significance to the churches or church groups who have completed the Survey. Our specialists are trained to evaluate each one of these Sections. Each church will have to decide if they are satisfied with the Standard Report or if they want additional analysis by our consultants. There is an additional charge for all enhancements. We will quote these on request. Page 1: Summary of Surveys in the Report. This shows a count of all of the Surveys and the weeks in which they were entered. Note: The Summary and Suggestions Report is NOT included here as it is a customized report prepared by our staff. In it we review each Standard Report and prepare a one page analysis of the major items we believe are worthy of additional comment. We also make some suggestions for the church to consider for additional help or enhancements to their Standard Report.
