SLAC Accelerator Department Type A Investigation and PEP-II John T. Seeman Assistant Director of the Technical Division PEP-II MAC Meeting December 13, 2004

SLAC Accelerator Department Time Line October 11: An electrical accident occurred in Sector 20 in the Linac klystron gallery which was a flash arc explosion in a 480 volt switch panel. A contract worker was severely burned and went to the hospital. He left the hospital about November 23 and returned safely to his home in West Virginia for continued recuperation. A one-day laboratory stand down was held on October 12. Employees since then have been slowly getting back to work as procedures and safety rules are updated and staff training occurs. DOE Type A investigation started October 18.

SLAC Accelerator Department Time line (Cont) SLAC will likely receive the Type A report the week of December 13. A DOE team of three Oak Ridge Regional Office people are to arrive December 14 to work with the local DOE site office and SLAC Management to produce a “Corrective Action Plan” and to help validate accelerator turn-on procedures. A proposed validation date for PEP-II and Linac is in the middle of January 2005.
