Verification After Testing Before submitting completed answer sheets, please ensure the following: All student names are filled in, legible, and spelled correctly Grade level is correct for each student Class ID is filled in and correct for each student Test bubble on answer sheet corresponds to test period on test booklet cover Subject box corresponds to the test booklet used for testing Student answers are bubbled in the correct question range corresponding to the item range in the test booklet Students should have separate answer sheets for each subject Note: For the Early Skills Assessment (ESA), schools should submit the entire student booklet for scoring using the boxes provided. Answer sheets will not be used for ESA. Each student has a unique student ID. If a student file has already been sent to Discovery Education Assessment, the student IDs used on answer sheets must match student IDs already submitted. Answer Sheets - 2 -

Student ID If an NCLB file was submitted, student IDs on answer sheets must match student IDs sent in NCLB file. Otherwise, your NCLB reports will not accurately capture the demographic data originally submitted. If you have not ordered NCLB reports, it is still recommend that you use the student ID to ensure that student data aligns across multiple testing periods. If different student IDs are used per testing period, data will still be scored but will not align across tests for comparison purposes. Using labels or pre-printed answer sheets can help prevent this problem. Answer Sheets - 3 -

Returning Answer Sheets Answer sheets are returned via UPS (United Parcel Service). If UPS does not come to your school on a regular basis, you have two choices: 1. Call UPS (1-800-PICK-UPS) to schedule an answer sheet pick-up. 2. Drop your answer sheets in a UPS drop box. NOTE: Please do not drop UPS envelopes in anything other than a UPS drop box. Please keep a record of your tracking number so you can track packages. Discovery Education Assessment cannot track your package(s) if you do not use the provided UPS envelope(s). Since Discovery Education Assessment will not have a record of the contents of your package(s), please check to ensure that all classes have been accounted for prior to mailing your shipment. Student Data Scoring Student data will be loaded into grade pools based on the class IDs used on the answer sheets. If the class ID field is incorrect or left blank, Discovery Education Assessment cannot guarantee that the data will be put into the correct report. Answer sheets will be processed within 10 business days of your return shipment to the scanner. The primary contact will be notified by that the reports are available online. A hard copy of reports will then be mailed to the school. REPORTS WILL NOT BE AVAILABLE BY INDIVIDUAL TEACHER UNTIL STUDENTS ARE POPULATED INTO TEACHERS’ CLASSES. TEACHER CLASSES CAN BE CREATED THROUGH THE ADVANCED FILE OR THE TEACHER WEBFORM. Answer Sheets - 5 -
