Five steps in community assessment 1. enlist community partners for assessment process 2. *define and analyze community 3. *develop community health profile 4. conduct the community assessment 5. mobilize the community

Define and analyze a community – two dimensions & specific indicators 1. Sociodemographic data – e.g., population figures (by age, gender, race/ethnicity), unemployment rate, poverty rate, educational attainment 2. Service information – e.g., police, fire, schools, health care services/setting, places of worship, social services, parks/recreation

Obtaining the data 1. Secondary sources: census.gov, infoshare, nsus/popbri.html nsus/popbri.html 2. Primary sources: surveys, observations, key informant in-depth interviews

Creating a community health profile 1. Morbidity data – e.g., prevalence of diabetes, incidence of HIV infection 2. Vital statistics – natality & mortality data 3. Behavioral data – e.g., smoking, sex with a condom, wearing a helmet while riding a bike, drug use

Obtaining the data 1. Secondary sources: reports from the CDC, the NCHS, DOHs; e.g., a/2000nhp-brooklyng.pdf 2. Primary sources: surveys
