E1 - Principal Self-Assessment The Principal Self-Assessment is a required element of the Principal Practice measure within Principal Evaluation. The Self-Assessment serves as a reflective guide for continued principal professional growth. It can also be a tool to support dialogue between Principals and Chiefs to deepen understanding of professional practice. The Self-Assessment may be used as a road map for guiding self-improvement and for developing individual professional goals.

A1 - Continuous Improvement Work Plan CIWP – NEW THIS SCHOOL YEAR! The Continuous Improvement Work Plan (CIWP) is the strategic planning process for schools that also meets the federal and state requirements of a school improvement plan. This plan establishes a school’s mission, strategies and milestones the school will take to accomplish its goals. The CIWP follows a continuous improvement cycle, where goals, strategies and milestones are monitored regularly and adjusted as needed.

A2 - Principal use of data to drive instruction School administrators are encouraged to facilitate data analysis at three interdependant levels within their schools: individual teacher, teacher teams, instructional leadership team. Data requires analysis - active questioning about practice and root causes to make informed decisions on best actions. Analysis should be timely, systematic, and systemic in order to have the greatest impact on instruction and strategy.

A4 - Medical exception compliance as of 10/31/15 The Illinois State Board of Education (ISBE) requires all enrolled Chicago Public School (CPS) students to provide proof of their required immunizations and health exams to the school by October 15 of each school year. All schools within District 299 (including Alternative, Charter, Contract, and Performance) are expected to reach 90% overall medical compliance by the state- mandated deadline of October 15. The goal of achieving the state-mandated 90% compliance target for our students is to prevent the spread of disease and outbreaks throughout the District.

A4 - School safety and fire drill plans School administrators must ensure compliance with the Illinois School Safety Drill Act to ensure that all school students, staff, visitors as well as first responders, are adequately prepared to respond in an emergency. School preparedness is created through effective planning and executing the following drills: Evacuation, Bus, Shelter- in- place, Lockdown/ Law Enforcement and Asthma/ Allergy.

B1 - School-wide Professional Development Plan Engaging staff in professional development aligned to district and school priorities provides crucial knowledge and understanding necessary for their work, and in turn contributes to a successful school year for teaching and learning. Professional development activities should be planned and aligned to the Continuous Improvement Work Plan (CIWP).

B2 - Principal feedback following classroom observation (3+ examples) A random sample of feedback from select observations that have been conducted by the principal and provided to the teacher will be selected for assessment of the principal's proficiency in providing appropriate and actionable feedback.

B2 - The Percentage of REACH observations completed The observation monitoring report provides Principals with information regarding the amount and types of observations that are required for each teacher. It also contains the number and percentage of observations that have been completed and are remaining as of the date of the report.

C1 - Compliance with IEPs ODLSS IEP/504 compliance data is based on the most recently completed annual reviews and evaluations for each student with an IEP or a 504 plan. Compliance data calculations are also cumulative across the school year. Students with annual reviews or evaluations due are included in the calculation of the school's IEP/504 compliance where the student was enrolled on the day that data was pulled from SSM.

C1 - 5Essentials In SY , the State of Illinois began requiring all schools to administer the 5Essentials survey. These online reports provide teacher, student and parent survey results about the culture and climate in their schools.

D1 - LSC or Alternative LSC Evaluation The power and duties of the LSC shall include evaluating the principal's performance annually during the entire contract term. The LSC evaluation, based on specific performance indicators, is submitted to the Principal and Network Chief by June of each school year.

D1 - “State of the School” slides and/or supporting documents Every CPS principal must conduct a State of the School Address for parents, LSC members, teachers, and community members in order to share the current state of the school including CIWP priorities, accountability targets and the strategic plan for school improvement. The State of the School Address should be conducted by November 24th. As part of this process, the principal must gather feedback from the local school community on educational priorities and needs.

D1 - Evidence of parent and other stakeholder engagement efforts, including examples of written communication A selection of artifacts which reflect the principal's family and community engagement efforts will be assessed. Evidence might include events such as Open House, curriculum or data nights, Doughnuts with Dad, Muffins with Mom, and written communications such as letters, newsletters, school blog or website posts.
