2 Overview Update At-A-Glance Preliminary Evaluations from Leadership Training LHD Survey Data -- Preview Health Homes and Health Departments

3 Medicaid Contract for Technical Assistance, Training, and Evaluation Services, KDHE-WSU Public Health and Medicaid Coordination Workgroup convened system partners Seven Workgroup meetings with resources posted to KALHD website Enhanced Coordination Programs presented to the workgroup: Diabetes Prevention Program, Lactation Support and MCH EBP, and Asthma Education Provided TA related to meetings with Health Care Finance and recommendations to state plan services Examine Expanded Evidence Based Practices Conducted five key informant interviews with five states about their CDSMP, staffing, and funding of those efforts through Medicaid Assessing needs and developing training/technical assistance to meet needs to expand capacity for CDSMPs Chronic Disease Self-Management Program Process: An application process, communications process, logistics, and curriculum design completed Two, Four-Session Leadership Series Trainings have begun; West—3 of 4 completed; Central -- 2 of 4 completed; 18 directors, six faculty and coaches, and a third planned for spring Leadership Development Provided materials about the Self-Help Network at five conferences Self-Help Support Resources Convened a Billing Workgroup to address concerns identified by LHDs Supported administration and analysis of statewide local public health department survey; assessing workforce, programs and accreditation readiness Other Activities Update through September 2013, Funding for this project is provided to CCSR through a KDHE Title XIX Medicaid Contract.
