Background The purpose of this first of a series of events is to describe the vision, ethos, service model and direction of travel for the Gwent Frailty Programme. It will also provide a genuine opportunity for staff from the various sectors to express their initial feelings, thoughts and suggestions on how we can make this work realistically in practice. It is aimed at frontline staff (i.e. those that face service users and the people that directly manage them) from existing Community Based Services, the Trust, Local Authorities, Local Health Boards, Voluntary & Independent Sectors.
