Procedures: Before and After 1.C&G officer requests disclosure forms (700U or FFD). 2.If the disclosure form is positive, C&G officer requests the addendum form. 3.All forms are routed to the COI staff for further processing and COIOC review. 1.C&G officer requests disclosure forms (700U or FFD). 2.If the disclosure form is positive, COI Analyst requests longer addendum form. 3.Form is processed by the COI Analyst and reviewed by the COIOC.
