Prospective patient is screened. Addiction problem? Yes Prospective patient is assessed to determine level of care and other services needed. Preliminary treatment plan is developed. Applications for entitlements are made. Does patient need case manager? Yes Assign case manager to patient. Refer patient to treatment program, recovery support services per treatment plan.
