2 Key ways that overall HIT Strategy will link to SIM Population Health HIT Healthcare Delivery System Payment innovation models Regulatory Stakeholder Engagement Quality Measure Monitoring and Evaluation State and federal innovation alignment * Already strongly supported by healthcare delivery system and payment innovation model sections Requirements for the Health IT plan includes (not exhaustive) 1 : ▪ How the specified HIT elements will achieve statewide health transformation ▪ Comprehensive plan to implement infrastructure ▪ Explanation of how governance will incorporate and expand existing public/private health information exchanges, including those operated by ACOs ▪ Policy and regulatory levers to accelerate standards-based HIT adoption ▪ Plan for promotion of patient engagement and shared decision making ▪ Plan to utilize telehealth and perform remote patient monitoring ▪ Plan to create electronic quality reporting ▪ How the state will provide technical assistance to providers 1 Current health IT effort will likely support some but not all elements of the plan required for SIM purposes

3 The inability of payers to rapidly identify new members based on medical history is often a missed opportunity… Albert suffers an asthma exacerbation leading to a 3 day hospitalization for intravenous steroid treatment Albert is a 25 year old freelance photographer in Dayton and is currently covered under his mother’s insurance Albert will soon turn 26, so he looked online and selected a plan in Cincinnati where he is planning to move Albert has a history of severe asthma and has had 3 ER visits in the last year; he has not filled the prescription for his rescue inhaler recently Albert’s new payer unsuccessfully reaches out to perform a health risk assessment; the payer does not have any medical history to use for risk stratification 2 Better access to historical data on new enrollees could have helped the payer identify a high risk patient and intervene
