Unplanned Event – MOF Propane Leak 3 February 7, Way #4, MOF. Replacement of a 2 inch SS isolation valve. Propane leak occurred at valve after line was in service. Appropriate weld procedure and certifications not provided. Understand requirements of the job. Provide documentation required by contract. Provide qualified personnel for assigned tasks.
