Required Paperwork Letter of Request Chain of Custody Record
Letter of Request Must be included with every case sent to MSL. Must be signed by someone with the proper signing authority (command- designated; “By direction”)

Regular, Priority, or RUSH Analysis? Regular – 3-5 business days Priority – Large expenditure of Oil Spill Liability Trust Fund or RP questions the extent of the spill (1-2 business days) RUSH – Detention of vessel/personnel or extreme public interest / media coverage (ASAP, outside normal business hours)

Chain of Custody MSL recommends that each unit have a designated Sample Custodian and a designated Assistant Custodian. “Relinquished” date and time needs to be the same as “Received” date and time. Use the same numbering system on the COC that you used on the jar labels.
Chain of Custody Samples must be: “Relinquished” to MSL on COC & “Shipped” to MSL in MISLE Tracking

Common Discrepancies Letter of Request not signed. Chain of Custody Record not the original. Chain of Custody Record “Relinquished” and “Received” times do not match. Samples entered into MISLE but not “shipped” to the MSL (tracking).
