Service Model Wrap Around Model stabilized the children, family in the community service provided to individual, group and family by a team of professionals including psychiatrist, LCSW, MFT, psychologist System of Cares home-based programme, focus on home-living prevent the removal of children from their home involve case manager, parent advocate, therapist

Service Model Contiuum of Care base on the severity of the problem of the consumers to provide different level of care Outpatient > Intensive Outpatient > Intensive Day Treatment > Hospital Preventive service develop early identification service for aged 0-5 develop assessment tools

Practice environment Participation of Prosumers employ the ex-consumer to be staffs involve the prosumers in leading the self-help group with close and intensive supervision by social workers acknowledge their experience and knowledge Caseload of the social workers Caseload is much fewer than HK School-based service 3:8 Intensive Day Treatment 1:10+
