Crew Names of team members and their roles in the film (director/co-director, treasurer, marketing, writer,music, props,etc) Name of Adult Ally’s and their relationship to youth team

Film Name Mission Statement – (This explains, in 1 or 2 sentences about what your video will do.) Example – Our film seeks to promote positive attitudes about teens making smart decisions against underage drinking. (Goals are what your intent is with this project, what do wish to accomplish with your message i.e. educate parents about the dangers of underage drinking) –Goal 1 (example) We will implement a youth led video project to increase public awareness for the dangers related to underage drinking –Goal 2 (example) We will host a video campaign through facebook and you tube to increase the reach of our targeted messages (working towards 1000 hits) –Goal 3 (example) Use the video to inform teens about resources available to help them make positive and healthy decisions
