The Clinical Governance and Performance Lead The clinical governance & performance lead is responsible for day to day monitoring of all aspects of service provision and delivery including mobilisation, clinical governance, information governance, auditing and performance monitoring of the service. Accountable to the Board of Directors Role is facilitated by OptoManager

New to the service - How do I sign up? Complete – PENE Optomanager Registration Form and send to Webstar health will send PENE a subcontract to sign, once signed this will be sent to you to sign once signed Webstar Health will send an E mail. Look out for an E mail from Webstar Health. This will guide you through how to set up access to the modules. Subcontractors you will need your QiO level one certificate AND your three QiO Subcontractor Checklists ready to upload. Performers make a declaration and upload their Glaucoma WOPEC certificates. Now you can input activity. IOPRR – IOP Repeat Reading Service PENE Community Service This collects all the data for the service Children Service – Optoclaim Services Recording This currently is for payments only – paper based system for reporting until new module ready.
Governance Subcontractor requirements : – Hold a GOS contract – QiO level 1 - GOS contract compliance Should be completed annually – submitted to NHS England every 3 years – PENE require this to be uploaded to the new module. Public liability insurance Clinical Governance – QiO NHS Standard contract compliance Subcontractor checklist – 3 lists required at commencement for new subcontractors & by 31 st March 2016 for existing subcontractors

For subcontractors with more than one practice The three checklists are completed and uploaded by the ‘Head Office’ branch. See ‘QiO Submissions for National chains and Multiples’ available from the NT&W LOC website Head Office communicates with CG&P Lead to advise this is the ‘Head Office’ branch and lists the other practices covered by the checklists.
Practice Details and Roles Statement Each practice within the group (except the ‘Head Office’) completes the ‘Practice details and Roles Statement' available from the NTW&LOC website. This is uploaded three times instead of the checklists. This must be done after the ‘Head Office’ has been signed off as compliant with the QiO checklists AND the head office has provided the list of the practices covered by the checklists to the CG& P Lead
Governance Performer requirements: – On-line declaration – GOC registration – WOPEC Certificate Required at commencement IOPRR – (WOPEC certificate for Children service available later this year) – DBS certificate subject to inspection by PENE – DOCET Safeguarding Children and Adults level 2 (or Hospital safeguarding to level 2 for children and adults) - subject to inspection. – Need to report complaints and significant untoward incidents

Performer Declaration By entering your name and clicking the "Agree" button you confirm that as the performer you agree to follow the clinical protocol as described in the contract head agreement. I declare: I am competent to deliver the Children Cycloplegic refraction service safely. All qualifying patients will be processed through this service according to the protocol and requirements of this service. A Crowded linear LogMAR test chart will be used in the provision of CRS I have completed safeguarding training to level 2 (Children and Adults). I have a current enhanced DBS certificate I will check all entries filled by someone else and enter the sign-off number myself.
