INVESTIGATING Incident Investigation steps: Respond promptly, collect information, analyze causes, develop remedial action, review finding, follow-up Information to include in an investigation report: Type, cause, & potential of loss, probability of occurrence, description of incident, recommendations Benefits of a proper incident investigation: Detailed information about incidents, trends identified, recommendations to modify dangerous equipment & tools and for better work practices NEVER part of an investigation or report Finding out who is at fault so that blame can be assigned Help other worksites to prevent incidents Provide information about types of injuries that occur most often & the most frequent causes

IMMINENT DANGER Any danger not normally found in the job, or a danger under which a person doing that job would not normally carry out the work
