3 LTCIP Strategies Reviewed n Managed FFS model: –MD focus group: May 19 –CMS Chronic Care Improvement Initiative n Fully integrated model: –AAP review; consensus June 9 PC meeting –Dr. Meiners, legislative committees and –SB 1671 allows for MassSCO replication –Budget bill proposes $.5M: Contra Costa & SD

Network of Care Strategy n AoA Aging and Disability Resource Center grant to California (2 counties) –Analyze current programs/processes –Identify modifications to enhance –Partner w/MDs, other professionals –Conduct public awareness activities –Target underserved, hard-to-reach
