Health and Wellness Options Together, we as the community members of central Michigan, including the counties of Arenac, Clare, Gladwin, Isabella, Osceola and Roscommon, will improve our overall health and promote wellness. Work together to improve the overall health and promote the wellness of the community members in the central Michigan counties of Arenac, Clare, Gladwin, Isabella, Osceola and Roscommon.

Health Options Together, we as the community members of central Michigan, including the counties of Arenac, Clare, Gladwin, Isabella, Osceola and Roscommon, will improve our overall health. Work together to improve the overall health of the community members in the central Michigan counties of Arenac, Clare, Gladwin, Isabella, Osceola and Roscommon.

Progress on County Health Improvement Planning OrganizationActions To DateSuspense Actions Clare County Human Services Coordinating Body Awaiting confirmation of being placed on the agenda for the June 25th meeting. 06/25 – VR to present if possible Isabella County Community Collaborative 06/04 – Unable to get on the agenda, so VR attended the meeting for informational purposes. 08/06 – VR to present if possible

Progress on County Health Improvement Planning OrganizationActions To DateSuspense Actions Mecosta and Osceola Human Services Collaborative Body 06/10 - Unable to get on the agenda, so VR attended the meeting for informational purposes and to request presentation time at August 12th meeting. 08/12 – VR to present Osceola County Board of Commissioners Have been placed on the agenda for the July 7th Board of Commissioners meeting at the suggestion of Larry Emig, Commissioner Chairperson and Mecosta/Osceola HSCB member. 08/12 – VR to present

7. Progress on County Health Improvement Planning OrganizationActions To DateSuspense Actions Arenac Multi- Purpose Collaborative Body 06/14 – VR attended mtg. They have created a Community Needs Assessment Planning Sub- Committee which meets after the MPBC meeting. It was agreed that they best route would be to have VR attend those meetings and for CMDHD to support that group. CMDHD has been given permission to invite people who are not members of the MPCB to take part in the assessment. Next meeting is on Aug 9 th at 2PM or immediately following the full meeting. 08/09 – VR and others willing to serve on the Arenac County Community Needs Assessment Committee to attend meeting. 7. Progress on County Health Improvement Planning OrganizationActions To DateSuspense Actions Gladwin County Human Services Coordinating Body 06/15 – VR presented. It was determined that there will be Executive Committee before the July 20th meeting to discuss whether to champion the effort, and they will present their opinion at the July 20th meeting. 07/20 – VR to attend meeting to find out the Executive Committee’s decision Roscommon Human Services Collaborative Body 06/16 – VR presented. The HSCB voted to approve a HIP working Group. Members will include representatives from: Mercy Hospital – Grayling, MidMichigan Health, Michigan Works, MSU Extension, Northern Lakes Community Mental Health, North West Community Action Agency, Roscommon County Transportation Agency, and CMDHD VR to coordinate with Cindy Dehadurantaye, HSCB Secretary, to schedule a meeting. It may not take place until August. Location to be determined.

8. Additional Health Indicators Additional county specific information packages are being prepared with data from the US Department of Health Community Health Status Indicators Report located at The packages contain information on  Demographics  Vulnerable Populations  Access to Care  National Leading Causes of Death by Age Group  Risk Factors for Premature Death  Measures of Birth and Death  Relative Health Importance  Infectious Disease Cases  Environmental Health

8. Additional Health Indicators CMDHD is in process of developing an additional set of approximately 40 indicators regarding mortality, morbidity, and health factors. The data will be by county.
