PRE HOSPITAL THROMBOLYSIS 2 days training. Assessment, then able to thrombolyse Use of a clinical advice line for verification of ECGs With excellent results. Only a small amount of patients are thrombolysed within B & H
PRE HOSPITAL THROMBOLYSIS Clinical advice line introduced Able to access clinical advice 24/7 Able to transmit an ECG if diagnosis is unclear or the patient goes outside of our criteria. Means more patients are able to be pre hospitally thrombolysed Also used to for Primary PCI patients. Good support for our crews
PRIMARY PCI In West Herts we have no thrombolysis. Since 2006 all STEMI patients go direct to the cath lab for angiogram/plasty. We use Hemel Hempstead cath lab Monday- Friday Harefield 24/7 In 2007 we transported over 200 patients.
PRIMARY PCI All patients will be moving to PPCI by April 2009 (hopefully!) Using Harefield, Royal Free, Papworth and during office hours Hemel Hempstead and Lister. The geography will dictate which hospital is used. No longer than 60 minute journey time under emergency conditions
PRIMARY PCI All patients with ST elevation. Under 12 hours of pain Will not include LBBB or any patients who have presented unconscious. Direct from home to PPCI centre Crews choice of hospital All patients will be followed up and audited.
THE FUTURE To have PPCI throughout the two counties Have no more Paramedic Pre hospital thrombolysis To have call to balloon < 2 hours To continue to train all staff in ST elevation recognition.
