What is TRANSFORM The TRANSFORM initiative brings federal, state, local and private organizations together to identify pertinent business challenges, foster collaboration, and deliver shared solutions. LOCAL - COMMUNITY FEDERAL PRIVATE (Provider, Citizen) Linking Health & Human Services Create efficiencies, reduce costs, serve our citizens STATE

Our Challenge – Current State Today, the focus is on the individual and their family to navigate the landscape of federal, state, local, and private programs available to address their needs. There is little coordination across programs. 3 SNAP Schools Head Start Local Child Welfare Newborn Metabolic Screening Newborn Hearing Screening IRS CHIP TANF School Breakfast Program Medicaid Child Care School Lunch Program Healthcare Providers The individual has to determine which programs apply to their situation and provide the same information repeatedly. This is not efficient or effective. 3

Our Challenge – Future State Enabling information sharing across programs results in improved services to the individual or their family and creates operational efficiencies. 44
