Collectively, the three Networks serve more than 73,000 patients receiving treatment in over 1,000 dialysis facilities and 42 transplant centers.

As you can see, Network 1 has the smallest patient and provider population served in the IPRO ESRD Network Program, with Network 2 as the median, and Network 9 having more facilities than Networks 1 and 2 combined. Vascular Access 2016 Baseline: September 2015 Goals: LTC decrease in sub-set by 2% Re-measure: September 2016 (data available December 2016) LTC Baseline LTC Goal NW 1 14.4% 12.4% NW 2 16.29% 14.29% NW9 16.14% 14.14% As of draft SOW baseline month will be changed to September 2014, versus previously using October data. Data now closing every 60 days—data received sooner and shared as received. AVF needs to increase by 0.08% per month with LTC decreasing by 0.17% monthly to meet objectives. Need to be at 67.05% in August to be completely on track, shy by 0.16% Need to be at 13.10% in August and are below this 32 facilities with >20% AVG rate/ 31 with >15-<20% AVG Rates so 63 total >15% AVG Rate

Long Term Catheter Reduction All Networks QIA: Long Term Catheter Reduction Criteria: Targeted facilities determined by those >10% LTC in September of 2015 Project Period: Baseline September 2015; Improvement by last day of 3rd quarter of contract year (September 2016) Selection and Requirements: September 2015 data to determine targeted facilities (available 12/10/15) Goals and Measures: Primary Goal/Measure: Decrease LTC rate by 2% in targeted facilities Summary description of activities reported on CMS monthly report

Listen. Feel. Daily Access Check (English | Spanish) Medically Unsuitable Second Referral Remember, Quick Action can help you. If an incident patient gets used to having a catheter, it will be easier for them to refuse a permanent access.
