Impact Measures Family Support meetings recorded on common care plans (FSM02 / FSM03) All plans are registered with the CAF Team

Impact Measures Key data is recorded and monitored Child’s details (name, dob, address) Lead Professional Meeting attendance / apologies Key needs and issues Gaps in services Level of Need Able to report on any emerging trends

Level of Need FSM02 (initial care plan) identifies strengths and needs under each of the 5 Outcomes and a level of need, per outcome, is agreed. Child/young person is given a final ‘score’ in the range of 5 to 20.

Level of Need FSM03 (review plan) reviews each of the 5 Outcomes and a new level of need, per outcome, is agreed. The new level of need ‘score’ is recorded and we are able to see if interventions are having an impact.

Impact Measures Changes to Levels of Need ‘scores’ enable us to see the impact services are having and to report on: Overall progress against each of the 5 Outcomes Overall progress of children/young people Trends (e.g. area-based, age-based, etc.)

Impact Measures Over a quarter of CAFs completed on over 13s come from William Beamont high school Examined some of their cases to review progress of individual young people Before poor attendance substance misuse parental conflict self-harming low attainment Score = 13 After attendance at 97% no longer using substances or self-harming improved relationship with parents applied to college Score = 5

Reporting on progress Designated performance management role within the CAF team Can support all partners with reviewing their progress and engagement with Family Support Health Youth Inclusion Support Programme (YISP) Schools
