LTCIP Strategies Developed to Support “Vision” Communication Strategy (Aging & Disability Resource Connection ADRC) Fully Integrated Health Care Strategy- HSD+ Physician Strategy (TEAM SAN DIEGO Initiative)

TEAM SAN DIEGO A community training created to meet the challenges of: People living longer with diseases/conditions An aging population Healthcare designed for acute conditions, but… Most healthcare dollars spent on chronic care Pressures produced by high cost of healthcare Individuals with chronic conditions needing more than medical care

TEAM SAN DIEGO Training is a mix of on line training modules and face-to-face classroom problem-solving. The online training has eight modules that establish a common ground of understanding among participants. The classroom training puts concepts into practice with team-building, discussion of tools/techniques, and establishing “teams”.
