Required reporting: – Post SWI – Final Use and Impact Report (aka, the Final Report) – ADP and Dairy Sales (milk and yogurt) Timing – to Funds Schools week of May 4 – Online report available Wednesday, May 6 – Due date: Tuesday, June 2 District applications – Streamlined format – one report covers all schools – Individual ADP and Dairy Sales required from each school – to districts week of May 11 – Online report available Monday, May 18 Appreciate S/R reminders to all funded schools Final Reporting 11
