PIT & FISSURE SEALANT

3/1 کودکان بین سنین 1 تا 3سالگی پوسیدگی هایی را در 3/1 کودکان بین سنین 1 تا 3 سالگی پوسیدگی هایی را در دندانهای شیری خود دارند. در این گروه سنی پوسیدگی های سطح اکلوزال به تنهایی 60% ضایعات را ممکن است شامل شود. (سطح اکلوزال 12% از کل سطوح دندانی را شامل میشود) سطوح پیت و فیشور 8 برابر در مقایسه با سطوح صاف مینایی مستعد به پوسیدگی هستند.

95% ضایعات و پوسیدگی در سطح اکلوزال سطوح پیت و فیشور 8 برابر مستعدتر به پوسیدگی هستند. 90% پوسیدگی های دندانی در کودکان پیش دبستانی در پیت و فیشور های سطح اکلوزال روی میدهد.

Preventive Dental Care

دو گونه اصلی از شیارها: I.شیارهای V شکل (کم عمق و عریض ـ خود به خود تمیز میشوند ـ تا حدی مقاوم به پوسیدگی) II.شیارهای I شکل (عمیق و باریک و کاملاً تنگ ـ حساس به پوسیدگی) در سطح صاف ← حداقل 1mm مینا بر روی عاج قرار دارد. در عمق یک پیت یا شیار ← مقدار مینا خیلی کم و یا نزدیک به صفر است.

آمالگام صاف کردن با فرز نیترات نقره با کلرید روی مسدود کردن پیت ها و شیارها (اچ کردن مینا و کاربرد ماده پلاستیکی)

No radiographic or clinical evidence of interproximal caries. Sealant placement Adequate isolation: place sealant. Not Adequate isolation: allow further eruption & place sealant within 1 to 3 month.

DiagnosisTreatment Caries - free surface: explorer wedging Explorer wedging due to pit & fissure anatomy stained or decalcified appearance of pit & fissures no radiographic or clinical evidence of interproximal caries. Sealant placement Adequate isolation: place sealant. Not Adequate isolation: place sealant in accessible fissures & seal remaining fissures after further eruption (within 1 to 3 month); or remove overlying tissue & place sealant within 1 to 3 months. Place sealant. Incipient caries: minimal involvement Export catch† due to incipient or minimal caries involving limited areas of pits & fissures indicative of incipient or minimal caries. Involvement of adjacent pit & fissure enamel with possible minimal involvement of underlying enamel and dentin. Preventive restoration placement (Restoration of Isolated Pit & Fissures) Preventive resin restoration (Sealant alone; sealant & filled resin) Sandwich preventive resin restoration (glass ionomer liner, glass ionomer restorative material & sealant)

DiagnosisTreatment No or minimal undermining of isolated pits & fissures No radiographic or clinical evidence of interpoximal caries. Possible radiographic evidence of occlusal caries. Sealant-amalgam preventive restoration (amalgam in isolated pits & fissures without extension for prevention & sealant) Glass ionomer resin preventive restoration.
