The integrated 2 -2 ½ year review in Warwickshire

What does it involve? The childcare provider completes a progress summary and shares it with parents. The parents complete the Ages and Stages Questionnaires which they have received from the Health Visiting team. The Health Visiting team or childcare provider complete a ‘WellComm’ language screen. The parents and child attend a health review appointment and take with them the Ages and Stages Questionnaires and the progress summary.

What resources are there? A letter for parents from the Health Visiting Team A poster for the childcare provider A flyer for the childcare provider A postcard for the health visitor team to give to parents

What happens and when?

The setting puts up a poster

The child has their second birthday

The childcare provider makes a note of which children are aged between 2 and 2 ½ and prepares to share the progress summary with parents.

The health visiting team send a letter to parents for the integrated 2 – 2 ½ year review appointment

The parents receive the letter. It requests that they complete the questionnaires and ask their childcare provider for the child’s progress summary.

The childcare provider gives the parent a copy of the progress summary and attaches the flyer. A copy of the WellComm screen results can also be attached if completed.

The parents attend the integrated 2 – 2 ½ year review with their child. A member of the health visiting team will discuss with the parents how they feel their child is developing And they will observe the child.

The parents take with them: the copy of their child’s progress summary their child’s red book the Ages and Stages Questionnaires which they have completed

A member of the Health Visiting team completes the review They complete the remaining health and development checks. They look through and discuss the Ages and Stages Questionnaires and the Progress Summary which parents have brought with them. They complete a ‘WellComm’ language screen if not already completed by the setting.

Any questions or concerns are discussed and the parents are given a postcard completed by the health visitor

The parents take the postcard back to their childcare provider The health visitor and childcare provider follow up any next steps
