2 © 2010 Map of Medicine Ltd. Commercial and in confidence. Re-approval of a care map A care map may require re-approval and publish for the following reasons: 1. A care map has been localised and needs updating before the date set for review 2. A care map has been localised and feedback has highlighted an error that requires changes being made before the date set for review 3. A care map is due for review – this process should start approximately 6 month prior to the set review date

Re-approval of a care map when changes are needed during the care map’s lifespan – Navigate to the care map where changes are needed – Create a draft of the care map then make the requested changes – Select the action submit pages for Review – Produce a PDF of the care map – Circulate to the care map to the reviewers and /or those requesting the changes – Once all agree with the changes review the care map in MMS – Submit the care map for approval – Follow the Approver 1 and Approver 2 process

Re-approval process when review date approaching – The publisher should have a record of when the care map is due for review and should alert the reviewers in good time to ensure that they are available to review the care map within the necessary timescales – Navigate to the relevant care map – Produce a PDF of the care map and circulate to the reviewers, these may be the original reviewers or new reviewers – Collate the responses and edit or pass to an MMS Editor to make the changes in the newly created draft care map – Re-circulate the update care map to the reviewers – Once all agree with the changes review the pages in MMS – Submit the care map for Approval – Follow the Approver 1 and Approver 2 process

Approval of care maps – Approval takes place at the care map level – During and after approval: – pages within a care map cannot be edited – an Editor cannot recall the pages for editing, unless an Approver sends the care map back for further changes – an Approver can do this by: – rejecting a care map (during approval) – recalling a care map (after approval)

First and second approval of a care map – Each care map must be approved by two different approvers before it may be published – The first and second approver have the same rights – the only difference is the chronological order in which they approve the care map – Second approval of a care map is essentially the same as the first approval – at the point of second approval, a validity date for pages may be set

Page validity dates – Each approved page should be given a validity (expiry) date – An Approver can set the validity date whilst carrying out the second approval of a care map – The second Approver will usually set this date – The validity date should be agreed by the clinical group who produce the care map or during governance sign-off – The date should allow enough time for review, updates, re-approval and publication of the care map to be completed – Example: a clinical group decides to meet again in 12 months to carry out a regular annual review of a new care map – so they set the validity date for 18 months
