8 COURSE ENROLMENT What if registration for courses is not possible through the Virtual Dean’s Office (VDO):

10 1.Check the course schedule for overlaps 2. a lecturer/visit the first class to collect lecturer’s signature on the Request-to-the-Dean form 3.Obtain an aproval of your academic coordinator (not required for IB courses, courses conducted with companies) 4.Submit the form to the International Centre by Monday, October 12 How to proceed the paper registration? Requests submited after the deadline will not be considered!

10 1.Check the course schedule for overlaps 2.Fill out Request-to-the-Dean form 3.Visit the Centre of Foreign Languages - CNJO (room 203, building G) 4.Have the document signed by SGH language instructor 5.Submit the form to the International Centre by Monday, October 12 Registration for a foreign language course Requests submited after the deadline will not be considered!

12 REQUEST TO THE DEAN form (registration manual): Provide 10-digit code, full title & group number for each course you want to add Specify the necessity for addition/removal of a particular course Collect a signature on the Request-to-the-Dean form or attach an e- mail* confirmation from SGH lecturer proving your admission Sign the form Attach your home university’s statement confirming that the course is crucial for your study program (not required for IB and foreign languages courses) You must attend courses and groups you are registered for on the VDO * format at SGH:

13 1.Finalize course enrolment 2.Submit the Learning Agreement (LA) 3.Once LA is duly signed, SGH will calculate the payment amount 4.Pay the tuition fee 5.Submit payment confirmation to the International Centre F REE - MOVER STUDENTS

R EQUIREMENTS TO FULFILL AT THE BEGINNING OF THE ACADEMIC TERM: Register the date of arrival at the International Centre Certificate of arrival – if required by your home university The earliest official arrival date: Learning Agreement Erasmus+ students – the form applicable for the Erasmus+ programme provided by your home university only NON-Erasmus+ students - either SGH Learning Agrement form or the one provided by home institutions 14

LEARNING AGREEMENT (LA) GUIDELINES : Content of the „declaration card” on the VDO must be reflected in the final learning agreement (LA) Initial selection of courses (Table A) need to be signed by the Student and the Sending Institution prior to the mobility period. Exceptional changes (Table C) to originaly proposed study program (Table A) can be made upon arrival at SGH Final LA is ultimately signed by the Respective Person on the side of SGH. Duly signed LA (a scan) is ed to student’s home university. Every student needs to pick up a hard copy LA from the International Centre once ed Each course recorded in the LA will appear on the Transcript of Records Submission of a Learning Agreement is mandatory for all exchange students as well as free-movers: - Bachelor students – 2 duly signed original copies are required. - Master students – soft copy LA (signed) sent to:

EXAMS examination period: January , 2016 exam re-sits: February , 2016 detailed examination schedule will be available 10 days before examination period early („zero-term”) exams need to be arranged individually and directly with lecturers at the begining of the semester. 17

R EQUIREMENTS TO FULFIL AT THE END OF THE ACADEMIC TERM: Sign the deprature list at SGH International Centre Certificate of departure - if required by your home university Certificate of attendance - if required by your home university MAXIMUM DURTION OF THE FALL 2015 SEMESTER: – (4 months 8 days) 18

TRANSCRIPT OF RECORDS A certificate reflecting your outcomes mailed directly to your home university in March, 2016 Students must clarify inconsistencies referring to their grades if such appear on the Virtual Dean’s Office no later than 1 month after the end of the exam period If grades are inconsistent, please, notify lecturers/SGH International Office or the Dean’s Office in due time - mistakes do happen! 19
