A ventilation curtain was hung from the roof bolt plates and extended across the last open crosscut. The ventilation curtain was rolled up by the victim to allow him to see the face area. Survey station (spad) No The height at the accident scene was 65 inches. The roof bolt spacing from the bolts to the right rib were 33 to 35 inches. The rock that fell and struck the victim measured 12’3” long by 25 ½” wide by 2” to 10” thick. The rock struck the victim on the lower back and pinned him to the mine floor.
