O VERVIEW OF HEALTH HISTORY Weight:157 Height: 5 foot 3 inches Adult illnesses: neck pain, back pain, hearing loss, fatigue, low energy Family health history: vertigo, mental illness, vision problems, Modes of relaxing: yoga

RESEARCH Stretching is important for people who have back pain because it can reduce pain and prevent injury (Malanga, G., & Walsh, M. C. (0014, April 22). This can reduce clients pain making everyday life easier There are many ways to injure your back but mostly it is caused by a sprain. There are many treatment options( like some pain medication or heat/ice packs) that can be done but if absolutely necessary, then surgery could be done. (Hawkinson, N. (2014, March 28). Knowing this can widen my clients knowledge of treatment options.

R ESEARCH CONTINUED Physical therapy is a great way to help your body heal and minimize pain. Physical therapy has active treatments and passive treatments. Passive treatments can include deep tissue massage, hot and cold therapies, Transcutaneous electrical nerve stimulation, Ultrasound. Active treatments includes different exercises for different things. Each person has different treatment plans to treat their condition better.(Hakinson, N. (2014, February 24) knowing this can allow my patient to seek other treatments if needed. Staying physically fit can drastically improve your back health. Also, if you are fit, you are less likely to injure your back doing everyday things. There are 3 steps to success. Step 1, have satisfactory aerobic fitness. Next, focus on your back muscles. Lastly, don’t stress your back. There are a variety of exercises that you can do to improve your back health like swimming and fast walking. (kolettis, G. (2013, February 6). This can give my client other options for treating her back pain, and make a plan for her back health improvement

T ARGET BEHAVIOR Reduce back pain

T HEORY (HBM) High perceived susceptibility: higher risk of back related injury or more sever back pain in the future Low perceived severity: has had back pain before, takes pain medication and visits chiropractor Perceived barriers: being motivated to exercise and take care of her back High self-efficacy: exercising and eating healthier, has a supportive family

T HEORY CONTINUED Visiting her doctor and going to the gym can help strengthen her back and relive her pain Increased perceived severity: is currently taking care of back but needs to maintain health overall Other ways to reduce back pain could include reducing stress, stretching, and relaxing Raise perceived benefits: relaxing,exercising every often, and continuing to care for your back will lessen the pain Stay positive and motivated, never give up

S MART GOAL My patient will lessen or eliminate her back pain by going to the chiropractors each week for 4 weeks. Also, my patient will go to the gym 3 times a week to strengthen her back muscles. She will know that her goal has been achieved when she feels little to no back pain.

I NTERVENTION HIGH LIGHTS Back pain lessened. Muscles not as tight as they used to be. Patient mobility increased.

RESULTS Patient went to the chiropractor each week Patient exercised 2-3 times a week Patient experiences less back pain and increased mobility

PERSONAL / PROFESSIONAL ANALYSIS Articles were useful for my understanding of the issue and the clients Client stayed focused on her goal and succeeded in reducing her pain Working with clients is more interesting than I had originally thought The behavior theories helped figure out a plan of action to help the client
