Risk Focus THEMIS 10/28/05 CommentsApproach & PlanRisk StatementRank, Title, Criticality 6 3. Probe Dry Mass Reserves <15% Criticality If: The Probe dry mass reserve is below the recommended margin of 15%, Then: Fuel propellant and delta-V capability is reduced and secondary science will be impacted. Med Mitigate: All mass changes approved at THEMIS Project level. 90% of Probe 1 mass has been weighed. Mass budget reserves is 7.7%. No changes to mass reserves 1.Insufficient Schedule Reserves Criticality If: The project cannot develop a robust resource loaded schedule for the production mode of delivering 5 spacecraft buses and 25 instruments, Then: The project schedule reserves will quickly erode. Mitigate: Organize schedule reviews with AETD Status and review project schedules on weekly basis Project schedule reserves is 22 days (1.1 mo/yr). First Probe bus will be delivered to UCB in mid November. Med Code 500 and APL are reviewing multipaction test margins. Mitigate: Work with Code 560 to develop appropriate diplexer acceptance tests. If: Diplexer is not properly tested for multipaction Then: Diplexer performance maybe compromised. 2. Diplexer Multipaction Testing Criticality Med

Risk Focus THEMIS 10/28/05 CommentsApproach & PlanRisk StatementRank, Title, Criticality 7 GSE modified so this anomaly can be isolated to either the GSE or transponder. Transponder #3 and #4 have completed ATP without repeating the failure Transponder #5 has initiated ATP Mitigate: Monitor transponders 2-5 for repeat of failure. Code 500 performing a system of communication system If: Transponder is the cause of the anomoly Then: Project schedule will be impacted 4. Lost of lock during ATP (non repeatable) believed to be a GSE anamoly Criticality Med If: The reliability for S/C post-mission disposal being less than.99 is deemed unacceptable, Then: Significant cost, schedule, and mission impact can result. Mitigate: Obtain early approval from NASA/HQ for the current reliability value. Swales incorporating Boeing/KSC launch vehicle data for final report. ODA rev B will be submitted 11/30. Low 5. Orbit Debris Analysis (spacecraft) Criticality Low 6. Insufficient support during Mission I & T Criticality If: UCB SSL has inadequate spacecraft bus support during mission I & T Then: Problem and failures during testing will impact project schedule reserve Mitigate: Develop a detailed resource (personnel, facility, GSE) loaded schedule for mission I &T. Obtain needed commitments from support groups (i.e. Swales, JPL, SSL etc.) Code 300 will provide additional QA and Safety support at both UCB and JPL. Code 500 is reviewing Mission I&T plan and will provide recommendations
