Goals of our monthly webinars Our goals for our MFP monthly webinars are:  To provide training on key topics  To better the lines of communication between you and us, the MFP staff  To seek feedback on how to better the processes of MFP/waiver services for our participants  To provide the opportunity to share successes and challenges as RTC’s and MFP transitions

MFP Transition Numbers As of today August 13, 2015  142 referrals have been received  40 participants have transitioned to date (15 in 2014 and 25 thus far in 2015) 2 transitions set to go in the next 2 weeks

Successful Transition Story  Staci Bercier with Spectrum in Butte served as the RTC.  Participant transitioned from a Butte nursing home to a Butte subsidized housing apartment in October 2014.

Other Items Can anyone share suggestions on ways to move a participants belonging from one community to another? Please make sure that it is communicated to me when your participant is hospitalized and the dates involved. The number of days will then be added to the end of their 365. When your participant leaves waiver services please send a copy of the signed Form 55 with the discharge date on it. CMS site review in July went well. Jeff and Ellen stated that they were impressed with our program; the work being done and the direction we are headed. We are waiting on their written report.
