2015 Incentive Metrics Very similar to 2014 Incentive Metrics, with a few tweaks: ▫PCS ED visit rate will be replaced with risk- adjusted Overall ED Visit Rate per 1000 attributed patients (due to move to ICD-10) ▫Childhood Immunization/Combo 3 metric on MDC dashboard to be added ▫Three process metrics to be deleted ▫Points to shift slightly 8

Helpful Hints for Optimizing Your Registry Submissions When sending registry data to MDC for Electronic Clinical Quality Measures, any version of the BCN Layout going back to the start of 2014 may be used. ▫Valid versions include: 14, 15b, 20, 21, 23, MDC asks for two types of records that BCN does not: Tobacco Use Assessment and Depression Screening, therefore there are no codes in the Layout for them. Please use “MTB” and “MDP” respectively. Service codes as defined in the specifications are preferable to other coding methods (e.g. LOINC). Their use allows MDC to track records in a consistent manner throughout processes and deliverables, which in turn streamlines issues and appeal resolution. Immunization records are not required as we receive this data independently from MCIR. However, POs are free to send us any and all data for their MiPCT patients. It is not necessary to subset only the record types listed in the specifications. Our process aggregates all data submissions and then extracts what is needed. Please do not send MDC data for any non-MiPCT patients; we are not authorized to accept that information and it may jeopardize timely processing of MiPCT data. 10

MiPCT Team Based Practice and Physician Organization Success Stories Suggestion - to align topic of Success Stories with the proposed 2015 MiPCT Clinical focus areas ▫Addressing social determinants of health and overcoming barriers ▫Patient registry and data support for population health ▫Integrating behavioral health ▫Integrating palliative and end-of-life care ▫Addressing appropriateness of care (e.g., Choosing Wisely program, etc.) However, not limited to MiPCT 2015 proposed Clinical focus areas Submission of stories may be completed by MiPCT Practice team members, MiPCT PO Leaders 13

New MiPCT PO and Practice Level Success Story Program – Opportunity to Earn 2015 Practice Learning Activity Credits “ 1 e. Practice Learning activity: Submission of formal best practice consisting of documentation that includes the underlying policy, process, workflow, data illustrating the implementation and improvement resulted from standardized tool, protocol and a contact person willing to take questions from other POs and practices (2 points for submission). In addition to the above at the request of MiPCT Leadership, presenting at a CM best Practice, PGIP Summit, MiPCT Best Practice Webinars and other venues (4 points submission invitation to present from MiPCT leadership and completion of presentation” The Learning Activity credits requirements for 2015 may be accessed: learning-activity-requirements/ learning-activity-requirements/
