Growth Potential and Customer Profiles Growth in business will rely on two factors: everyone requires healthcare and Americans want everything “quick” Open 7 days/week with prompt care and no appointments necessary Target population is all age groups and will more specifically target people who need care after hours Cash only business but costs will be similar to that of office visit copays

Direct Competition Two direct competitors in Jackson County – Holton Community Hospital/Family Practice Associates which have the emergency room and after hour clinics but clinics require appointments and emergency visits are costly. Both may have waiting times – Holton Family Health Clinic which has after hour clinics but again require appointments and may have waiting times

Is within a 20 mile radius of most residents of Jackson County Will have extended hours and open seven days/week Owners are members of the community Owners are the providers and have a vested interest in providing professional, friendly and efficient care

Company Ownership Owners/providers are Jill Collins, Jessica Shirk and Brandy Schnacker who are all advanced practice nurses. Will be a Limited Liability Company (LLC) Initially will be staffed by volunteer family members and will eventually add staff as volume and income grows Providers are licensed by the state of Kansas as advanced practice nurses and maintain certification in basic and advanced cardiac life support.

Company Location and Facilites Located at 2304 W. 4 th Street in the downtown business district of Holton Equipped with 3 exam/treatment rooms, reception/waiting area, 2 private offices a small lab and public restrooms Easily accessible parking Handicapped accessible building, restrooms and parking Within 1 mile of ambulance service and 5 miles of a hospital in case of emergent situations

Operational Projections Plan to see approximately 8 patients per day the first six months with and average $50/visit By the seventh month, will be averaging 10 patients/day with the same $50/visit average By the second year, will be averaging 15 patients/day and continue to average $50/visit Will pay ourselves minimally for the first couple of years. We will remain employed elsewhere to maintain salaries and health insurance. Plan to add additional employees with employee only benefits as income and client base grow May eventually accept insurance and potentially grow into a larger building and add x-ray/ultrasound services
