Page 2 Requirements for Basic License 6 Points of Identification Vision Test: Minimum of 20/50 Vision Knowledge Test: –Consists of 50 Questions –Minimum of 80% or better

Page 3 Out of State or Out of Country Applicants Out of state17 and 18 year olds will be given a probationary license for a period of 1 year Out of country applicants must pass knowledge test and vision screening and may be required to pass a road test.

Page 4 Road Test Must have a minimum of 6 months supervised practice prior to road test appointment!
