Personally Identifiable Information includes the following: –S–S–S–Student’s name –T–T–T–The name of student’s parent/guardian or other family member –T–T–T–The address of the student or student’s family –A–A–A–A personal identifier, such as the student’s SSN or MOSIS # –A–A–A–A list of personal characteristics that would make the student’s identity easily traceable –O–O–O–Other information that would make the student’s identity easily traceable.

Directory information may include: –Student’s name –Major field of study –Participation in officially recognized activities and sports –Weight and height of members of athletic teams –Dates of attendance –Degrees and awards received –The most recent previous school attended –Photographs – A media release must always be signed before any photos are taken students.

CAUTION… When discussing student situations with other teachers – even when you are seeking professional advice – do not use student names – if the person you are talking to repeats what you have said about a student, you are the person who is legally liable because you shouldn’t have disclosed that information to begin with – the person who repeated it isn’t the one in legal trouble!
