Weekly Logs and Hours…  Fill out completely  Four-credit students do not include lunch break in total of hours  Use the Comment space on your Weekly Logs to set goals for yourself for the next week and to communicate your willingness to learn to your supervisor  Supervisor must sign after you have completed the front page  Complete Communication Journal on the back  Attach Reason for Absence or Request for Absence form and attach if applicable  Hand in on time (every Tuesday) The Weekly Log is the official record of your attendance in your Co-op course. Continued neglect to submit logs may result in your withdrawal from the program.

Absence Reporting

Arrival and Departure times from Co-op are to be regular Arrival and Departure times from Co-op are to be regular.  You must have permission from both your monitoring teacher and your placement supervisor before changing the hours you have contracted to work.  Placement times that are less than 3 hours must be approved by the monitoring teacher.  Changing the Work Ed Agreement MUST be approved by your Co-op teacher for the hours to count. Do not agree to work these hours without informing your Co-op teacher FIRST.

Making up hours lost through absence Try working extra minutes a day until you have caught up. The EFFORT you make to catch up short hours is as important as the time you actually make up. Morning Co-op students: if you are cutting your start time close at your placement, take an earlier bus.

Making personal telephone calls and texting from your placement is not acceptable! Wait until after your time of work is finished.

Accident Reporting: If an accident occurs at the placement you must: a) Get First Aid b) Report to your Placement Supervisor c) Report to your Monitoring Teacher

I don’t like your Attitude… Taking Responsibility Taking Responsibility for your own learning at Co- op. In your assigned group, focus your discussion about your Co-op placement experiences and/or the people you work with on the following topics: Attitude and Initiative: If you're tired, or stressed out from things happening in your personal life, try to maintain a positive impression. Complete the Attitude Inventory.  How did you score? Reflect carefully on how you have answered the questions. How do you think your attitude at your placement affects how your supervisor/co-workers/clients treat you?

Group Discussion Time People with positive attitudes tend to be optimistic, cheerful and outgoing and they see life as exciting, worthwhile and enjoyable. They usually get along well with other people and are happy most of the time. People with negative attitudes complain a lot and are often angry and withdrawn. They are unhappy a great deal of the time and they have a difficult time getting along with other people.  What is your attitude saying about you?  What is Initiative? Timer

Reflective Journal Writing Time Journals should be approx. 1 page in length and will be evaluated according to the rubric in your duotang. Complete Reflective Journal #4 writing time: Taking Initiative. Hand in by end of in-school today. Timer

Ontario Skills Passport This is a requirement for all SHSM students and is portfolio requirement for all Co-op students.  Go to the Ontario Skills Passport website (google Ontario Skills Passport) Ontario Skills Passport  Match your placement experience with National Occupations Classifications skills lists to print a personal work record. (See handout and Lanschool presentation).
